Going home checklist

Training

• Two people at home trained on every device and routine
• Practiced a night of care before discharge (rooming-in)
• Know the signs of trouble and what to do

Equipment and supplies

• Main equipment delivered and tested at home
• Backups for anything life-sustaining
• At least a week of supplies
• Supplier's 24 hour number saved

Nursing and help

• Nursing hours approved, and how many
• Agency named, and the first shifts staffed
• A plan for shifts that are not covered

Home

• Enough outlets near the bed
• Power company told, medical priority form sent
• Space for equipment and supplies

Appointments and papers

• First follow-up visits booked
• Written care plan: medicines, feeds, settings
• Emergency information form filled in
• Prescriptions filled
